feat(backend): assemble and run the backend from environment settings

Details

Main reads GATEWAY_URL, INFLUX_URL, PORT and the two timeouts, fails fast
with a clear message, and wires adapters and services. The end-to-end test
runs it against stub gateway and InfluxDB servers on localhost.

The test-only web/index.html is removed: it shadowed the real UI on the
test classpath, so ApiServerTest now checks the real index.html.
